We are a countywide, independent charity, working with rural communities in Northamptonshire to improve their quality of life. Through community engagement and development we support communities to reduce rural isolation and vulnerability and work with them to create a vibrant, sustainable future for all. We are one of 38 Rural Community Councils working across England, making up the ACRE Network

The Centre Project aims to reduce isolation and promote well-being. Current activities include- My connect ' Community Hub'- Centre Kitchen - Social eating- Support, information, advocacy, signposting and referrals- Day services for adults with learning difficulties- Activities such as, games, art therapy, day trips and IT classes- The Freedom Youth Club - Community Food bank

Sunday mass services, Holy Communion, Holy mass once a month on St Gabriel's day, yearly annual anniversary, baptizing children and adults, wedding Solemnization, memorial services, Befriending and counselling support for the elderly, visiting the elderly and the sick, bible Study, choir rehearsal every Saturdays, Sunday School conferences and periodically fundraising event.

To develop capacity and skills of the members of socially and economically disadvantaged community of West Howe. The promotion of social inclusion amongst people who are socially excluded from society as a result of mental ill-health. through the provision of creative events which foster social connection and allow for the exploration of mental illness, recovery and well-being for all. The main activity is the Dragon Cafe open Mondays in Southwark with other activities Recreate Psychiatry emerging from it.

Home-Start South Warwickshire provide practical help, friendship and emotional support to families in South Warwickshire where there is a child under five. Families may be struggling with parenting, physical or mental illness, disability, multiple births, loneliness or isolation. Our volunteers, who are parents themselves, undergo 30 hours of training, regular CPD and supervision.

YOH is a community led youth provision based in London and Essex, supporting young people who are marginalised, vulnerable or at risk of social exclusion. We aim to support the development of beneficiaries' aspirations, skills and confidence to pursue positive life ambitions and achieve economic independence, as well as providing a platform for our users to be heard and affect change locally.

To relieve the effects of poverty and social deprivation in North Wiltshire by collecting unwanted furniture and domestic appliances and redistributing them to individuals and families who are in conditions of need, hardship or distress.

The Spitz Charitable Trust aims to relieve isolation in local communities through the power of live music. The charity works with professional musicians to engage older people in care homes and residential housing through participatory music sessions, and also link them with young people to create a positive impact on the lives of everyone involved.

Communicare in Southampton is a community care group with volunteers, many from local churches, that works on behalf of the whole community in Southampton. It complements the help available to deprived people by visiting and befriending the lonely,elderly and those with mental health issues, shopping for/with clients, carrying out simple DIY tasks, helping with childcare and providing transport.

Fresh Start Charity provide work experience to those classified as NEET (not in employment, education or training) in our shop based in Ipswich. We provide training in Retail, Hospitality, Customer Service & Warehouse management and help them to manage their mental health in the work place. We also provide a welcoming cafe for the community to reduce loneliness and provide a Community Fridge.

Providing Health and Social Care throughout Fairford, Lechlade and the Surrounding Villages. Our services include End of Life Home Nursing care, Patient Transport for medical related appointments and a weekly Lunch Club for the isolated and lonely. We provide funding to various local community groups. We have a Charity Shop based in Lechlade.

King's Cross Methodist Church is a city centre church that seeks to be welcoming and hospitable to all. There are services in Cantonese, Mandarin, and in English. We organise support for refugees and asylum seekers and run a night shelter for homeless people during the winter months and a weekly social club for lonely people. Many community groups use our premises.

We aim to prevent social isolation and enable people to progress. The main objective of the West Norfolk Deaf Association is to work with deaf and hard of hearing people to facilitate independent living and reduce isolation.We aim to raise awareness of the issues facing deaf people and provide a community centre for deaf people to promote social, community and educational activities.We primarily cover West Norfolk but are able to extend our service as required.
