Transitions UK supports vulnerable young people, helping change lives and futures. We work with 14-25 year-olds using a blend of befriending, mentoring and social & educationally based opportunities for personal development. Key groups include young people with learning disabilities; those with mental health and emotional needs; young people at risk of exclusion/offending and those leaving care
