THE ACTIVITIES WILL BE UNDERTAKEN BY A SPECIALIST TRAINED MULTI-DISCIPLINARY TEAM WHO WILL PROVIDE SYMPTOM CONTROL, ASSESSMENT, TERMINAL CARE, ONGOING EMOTIONAL AND BEREAVEMENT SUPPORT + EDUCATION

Care for life threatened/limited children and young people and their families, through providing:* Respite care: day care and overnight stays* Palliative care at end of life* Bereavement suite and support* Community and hospice at home services: Physio, Music therapy, Toy loan, Special yoga * Family Support: Siblings, Counselling, Family Suite and Training to equip parents

St Mary's Hospice is the dedicated provider in South Cumbria of comprehensive specialised supportive and palliative care for people with life limiting conditions. Facilities and services are available to all adults who are facing the end of life, and to their families, carers and the bereaved.

St David's Hospice is an adult palliative care facility with fourteen registered beds providing symptom relief, respite and end of life care. There is also a day unit for up to ten patients four days per week, and a bereavement and counselling service for one day per week. All care is free at the point of delivery.

Treetops Hospice provides Daycare, Complementary Therapy and Hospice at Home care for people suffering from life-limiting illnesses. We also provide Carers Support and Complementary Therapy for people involved in caring for people with life-limiting illnesses and Bereavement Support for people and families who have suffered a bereavement.

CRY provides bereavement support to families affected by a young sudden cardiac death (YSCD/SADS) & supports young people with heart conditions. It raises awareness, runs a national heart screening programme to identify conditions such as cardiomyopathy, ARVC, Brugada, WPW, HCM, DCM & Long QT, & funds medical research, specialist NHS inherited cardiac services & expert cardiac pathology.

Care for the Family's aim is to strengthen and support family life. We provide relationship education and support in three key areas: marriage, parenting and bereavement. At the Jessie May Trust we provide a free Palliative Care Service for children and young people who are not expected to live beyond nineteen years. We work in Bristol, North Somerset and South Gloucestershire.Members of our experienced care team support each family at home with short periods of respite, advice, terminal nursing care and bereavement support.

We offer clinical care, support and advice to adults with cancer and other life limiting illness from North East Worcestershire area. In addition, we offer support and advice to the families, carers and friends of patients and a bereavement counselling service that is BACP accredited.
