Objectives are to:1) prevent and relieve hardship and need and distress amongst those who are homeless and rootless by the provision of practical and emotional support, in particular but not exclusively to those who are sleeping rough or who are unwilling or unable to accept existing provision;2) educate the public concerning the problems of social isolation and homelessness.

When asked, most of us say we want to die at home, in the comfort of familiar surroundings close to those we love.The Lawrence Home Nursing Team gives people and their families the support they need to make that possible. Providing free hands-on care, day and night, our nurses help to fulfil the wishes of anyone who wants to die peacefully at home.

The relief of sickness and suffering amongst people living in the North Ceredigion area who are living with advanced incurable and progressive illness and also their carers, by providing them with practical, social and emotional support, by providing community-based supportive end of life care and by raising awareness regarding all aspects of end of life care in the North Ceredigion area.

The principal objective of the Charity is to provide almshouse accommodation for persons who meet its entry criteria which include provisions relating to age, financial resources and acceptance of Christian values.

The charity supports individuals and family carers following brain injury across East Sussex, Brighton & Hove & West Sussex. This includes individual and group support for wellbeing and reablement at home and in the community, information and signposting including at our centre in Newick, East Sussex. CMCA Ltd is a community resource centre providing advice and information on welfare rights, housing, immigration and nationality laws individual support sessions and advice on education, training and employment opportunities. TO PROVIDE TRANSPORT OPTIONS TO PEOPLE WHO FIND PUBLIC TRANSPORT DIFFICULT TO ACCESS DUE TO AGE, DISABILITY OR GEOGRAPHICAL ISOLATION. TO AID OTHER CHARITABLE ORGANISATIONS AND TO HELP MINIMISE SOCIAL EXCLUSION BY PROVIDING AFFORDABLE MOBILITY ACCESSIBLE TRANSPORT. TO PROVIDE WHEELCHAIR LOANS AND TO PROVIDE TOE-NAIL CUTTING SERVICES AT AN AFFORDABLE COST, OFTEN IN CLIENTS HOMES.

Housing Matters (Bristol) provides free, independent and confidential advice to anyone in housing need. Through a combination of advice, support and advocacy we enable clients to resolve housing issues, improve housing conditions and prevent homelessness. Previously known as Churches Housing Aid Society, we have been providing this much needed service since 1965 (previous charity number 233757).

Calderdale SmartMove's objective is to find vulnerable and homeless clients in Calderdale (over 400 individuals and families each year) private rented accommodation then offer up to 12 months floating support.We are the sole organisation in Calderdale to offer a Bond Guarantee Scheme.Additionally, we offer free informal education for our clients, funded from external grants and donations

InCommon's activities bring together people of different ages. We directly deliver intergenerational programmes in Greater London with young people (age 5-15) and older people usually living in retirement housing. We also support other organisations and individuals to run intergenerational projects in England through advice, resource sharing and an online platform.

Home-Start is a voluntary organisation committed to promoting the welfare of families with at least one child under five years of age. Volunteers offer regualr support, friendship and practical help to families under stress in their own homes helping to prevent family crisis and breakdown.

AVSED is a local charity working to prevent social isolation & loneliness for Aireborough (LS19 & 20) seniors by supporting happy, healthy & independent living. We provide activities (physical exercise, social & friendship, health groups inc dementia, inclusion & lunch club and trips) alongside services (transport, needs assessment, handyman, befriending, digital & home support).
