TO PROVIDE A SPECIALIST PALLIATIVE CARE SERVICE FOR PATIENTS WITH SPECIFIC LIFE-LIMITING ILLNESSES LIVING IN THE AREA COVERED BY WREXHAM HOSPITALS AND TO SUPPORT THEIR FAMILIES IN A CARING ENVIRONMENT.
