To provide psycho-social and counselling, advice, information and support to help people affected by substance misuse to make positive change. The charity operates at tier 2 and 3 of the treatment system as defined by models of care.

To provide the best in palliative care by providing an appropriate and comfortable environment, well trained and sensitive staff to address the needs of patients and those around them. We work closely with colleagues in the primary care and hospital settings. Innovative practice is important to the hospice and we look forward to developing End of Life programmes to embrace a range of diseases.

Treetops Hospice provides Daycare, Complementary Therapy and Hospice at Home care for people suffering from life-limiting illnesses. We also provide Carers Support and Complementary Therapy for people involved in caring for people with life-limiting illnesses and Bereavement Support for people and families who have suffered a bereavement.

St David's Hospice is an adult palliative care facility with fourteen registered beds providing symptom relief, respite and end of life care. There is also a day unit for up to ten patients four days per week, and a bereavement and counselling service for one day per week. All care is free at the point of delivery.

SCS provides counselling and psychological therapy services in and around the City of Sunderland and the North of England to relieve suffering and distress, due to bereavement, loss or other life issues. We provide counselling and talking therapy services for adults, young people and children, and specialist services for survivors of rape and sexual abuse and people with life limiting illnesses.

THE RELIEF OF MEN, WOMEN AND CHILDREN WHO HAVE BEEN THE VICTIMS OF TRAFFICKING BY PROVIDING OR ASSISTING IN THE PROVISION OF SAFE HOUSING, MEDICAL CARE, COUNSELLING AND OTHER SUCH SUPPORT ANYWHERE IN THE UK OR OVERSEASTO ADVANCE PUBLIC EDUCATION ABOUT THE PLIGHT OF MEN, WOMEN AND CHILDREN WHO HAVE BEEN TRAFFICKED

The charitys objects are the relief of sickness and injury and protection of human life by providing and supporting an air ambulance service for the county of Wiltshire and surrounding areas.

St Andrew's Hospice provides specialist palliative care for people of all ages who have a progressive life limiting illness and also provides support and advice for their families and friends. We provide care for adults living in North East Lincolnshire and for children living in N E Lincolnshire, N Lincolnshire, Lincolnshire, Hull and East Riding.

The Charity provides sheltered flats for older people in Gloucestershire as well as a Residential Home and Nursing Home. The Charity also offers day care services for those living with dementia in the wider community as well as a weekly social club.The Trust also administers a "Relief in Need" grant making branch of the charity and the former Gloucester District Nursing Charity.

The Foundation's vision is to defeat lung cancer to be achieved by four major areas -Funding a research programme-Prevention activity inc stop smoking support, youth smoking prevention-Support activity inc practical and emotional suport for people affected by lung cancer-Influencing and campaigning to ensure patients have equitable access to the best treatment and care.

Provision of care for the older people who require permanent nursing care over and above general care.

Provision of the highest level of symptom management and care for people who have advanced active, progressive and life threatening illness., through the provision of a Specialist Palliative Care Team. The Hospice endorses a holistic approach in promoting quality of life for patients as well as providing the continuing support that is offered to families and carers.

The Charity provides accommodation and care for defined categories of men and women (the Brothers) in its historic buildings in Charterhouse Square in the City of London. It also provides financial support to Charterhouse School and has the power to operate a museum.

Delta UK has two central, though broad and interconnected objectives: to eliminate poverty and to advance education. We believe these are inextricably interlinked, that the first will not be achieved without the second. Without advancing education, at all ages and stages of life, we will not be able to offer more than palliative care and a band aid to communities unable to help themselves.

We provide palliative care for the people of Burnley & Pendle (and Rossendale for inpatient facilities) suffering from cancer and other life-limiting illnesses. We also provide support for their relatives and carers. We do this primarily with our 10 bed inpatient unit, our day service and outpatient facility and our Hospice at Home team who look after patients in their own homes.

Our mission is to impact the health of mothers by putting them at the heart of improving reproductive, maternal newborn and child health. Our Mentor Mother Model empowers mothers living with HIV, through education and employment, as role models to help other women access essential services and medical care.

Provision of tenanted accommodation,support staff, training & rehabilitation services, learning & social activities for children & adults. Equipment training & provision including, Information technology devices. Comprehensive advice and guidance, living with visual impairment including other disabilities specialist training for care workers, companies & other services. Community services.

Support people with Mental Health and Learning disabilities

The charity runs Activity Centres, Community Clubs and activities, Information and Advice Services, including home visits and a money advice service; Help at Home Services; Home from Hospital Support, Hospital Take Home & Settle, and Relative Support services, Community Support projects, Social Prescribing, Wellbeing projects throughout West Sussex, and Carer Support Services.

The charity operates to provide on-going funding to Sir Michael Sobell House (The Hospice). The fundraising effort concentrates on four main areas, which are:-- The Sobell Lottery- Community Fundraising- Financial Support - legacies, investments and grant income- Trading - shopsFunding is provided to The Hospice by way of grants and services provided.

The charity operates a non-maintained special school concerned with the education of deaf pupils of 3 to 19 years of age

THE OBJECTS OF THE CHARITY ARE TO PROVIDE EDUCATION, ADVICE, GUIDANCE AND TRAINING TO CHILDREN AND YOUNG PEOPLE WHO ARE BLIND OR PARTIALLY SIGHTED AND TO PROVIDE SUCH SERVICES AS CHILDREN AND YOUNG PEOPLE WHO ARE BLIND OR PARTIALLY SIGHTED MAY REQUIRE TO LEAD INDEPENDENT LIVES WITHIN THEIR OWN COMMUNITIES OR OTHERWISE MEET THEIR NEEDS.
