A HOLISTIC RANGE OF SERVICES THAT ENABLES PEOPLE LIVING WITH HIV TO PROMOTE THEIR HEALTH AND WELLBEING, SUPPORT SELF MANAGEMENT AND REGAIN THEIR INDEPENDENCE. THIS IS DONE THROUGH ADVICE, SUPPORT GROUPS AND TRAINING. ADDITIONALLY WE RUN PREVENTION PROGRAMMES TO PROMOTE TESTING AND SAFER SEX PRACTICES TO REDUCE ONWARD INFECTION. THIS IS DONE FROM OUR BASE IN EAST LONDON.

St Johns Winchester is dedicated to transforming the lives of older people. Our almshouses offer safe, affordable homes and a nurturing environment. Our dementia support service provides guidance, care, and social connections for individuals living with dementia and their families. Our Hand in Hand programme combats loneliness by connecting older people with local befrienders.

To provide advice, support and other services to empower and develop anyone experiencing a mental health problem in Bedfordshire, Luton and Milton Keynes

To provide convalescent, nursing and respite care for the sick and aged in the Sheffield area, through the provision of fully-serviced nursing accommodation for residential patients, and through one self-contained residential flat providing sheltered accommodation for the elderly, being fully registered with all required regulatory bodies.

Wellbeing/educational programmes in woodland and green spaces, helping disadvantaged adults and children refocus and rebuild their lives; engaging them in skills learning and self development: NEET, victims of crime, ex-offenders, mental health, substance misuse, ex-forces, unemployed, disengaged from school, learning disabilities. Conservation of the environment. Train and deliver forest school.

We help children access potentially life-saving treatment, whilst supporting research that not only aims to introduce new treatments but also bring existing internationally available options here to the UK. We support parents by sharing the most current resources available for neuroblastoma treatments and providing access to other parents who want to share their knowledge and experiences.

Age UK Camden is the legal name. The charity: (a) Provides services; (b) Works in partnerships; (c) Campaigns with/for older people; (d) Innovates and undertakes research. Current services include: Day Services; Information/Advice; befriending; social prescribing; counselling; shopping; computer training; support for older LGBT people.

The charity supports a wide range of charitable and health related activities benefiting both patients and staff across Bedfordshire Hospitals (incorporating both Bedford Hospital and Luton & Dunstable Hospital) that the NHS is unable to provide. Charity 1061003 Bedford Hospital Charitable Fund was amalgamated into this charity in 2022 in line with the merger of the hospital's corporate trustee

North Staffs Mind works to create a better life for everyone with experience of mental distress, primarily in North Staffordshire. The organisation has three main service areas providing support to individuals in various ways: counselling for adults, children and young people, and parents; supported housing; and training and development activity to raise awareness of mental health issues.

Free, confidential and independent advocacy support for vulnerable people to take part in decisions relating to their health and social care. The charity delivers a range of services including training, statutory mental health and mental capacity advocacy & general advocacy support for issues relating to health & social care in particular for those whose primary issues relate to mental health.

WE PROVIDE LIFELONG LEARNING AND SUPPORT FOR MORE THAN 100 PEOPLE WITH LEARNING DISABILITIES, AUTISM, PHYSICAL DISABILITIES AND MENTAL HEALTH CHALLENGES.IN OUR SKILLS TRAINING CENTRE WE PROVIDE ACCREDITED LEARNING AND INFORMAL LEARNING ACTIVITIES DAY CARE SUPPORT. IN OUR RESIDENTIAL CARE HOMES, BRACKENLEY AND BRIARDENE, WE SUPPORT 26 PEOPLE WITH LEARNING DISABILITIES.

Guideposts services are provided in England, mainly in Oxfordshire, Hertfordshire and Gloucestershire, and focus on improving the health and wellbeing of people with learning disabilities and autism, mental health challenges, children and young people, older people including those with dementia and their carers. Delivered via local hubs, community outreach, online and telephone.

Promoting the welfare of patients and staff of Salford Royal NHS Foundation Trust and the support of medical education and research within the trust.

The Advocacy Project ensures that London's most vulnerable people have meaningful choice and control over what happens in their lives. We support people living with mental health conditions, eating disorders,learning disabilities, dementia. We work with all age ranges from children and young people, adults and older people. We provide 1:1 advocacy, group advocacy and user involvement/engagement.

Penny Brohn UK is the cancer health and wellbeing charity. We help people feel better in mind, body and spirit by offering everybody with cancer a choice of personalised cancer care. All free, safe and backed by research.

The Foundation's grant making is bridge building to connect people and communities by bringing them together for the good of society, and ultimately to help create a more comprehending and cohesive world. The Trustees consider projects in areas covering the arts, health and wellbeing, education - in its widest sense, and social issues.

The principal activity is the running of a convalescent home and nursing home. There is also provision of pastoral and spiritual support.

The Multiple Sclerosis Trust is a UK charity for people with MS, their friends, families and carers and for the health and social care professionals who work with them. Promoting the mental health of professional musicians and those who work in supporting roles across the breadth of the music industry and those who have worked in such roles, by providing support, information, counselling, guidance and signposting to additional sources of assistance.

Broadreach House provides residential, rehabilitation and community based treatment services for alcohol and drug dependent men and women, offering high quality treatment programmes.

Charity based in Northern Warwickshire offering Wellbeing Day Hospice, Hospice at Home, Rapid Response, Lymphoedema and Family Support & Bereavement Services to patients, from Nuneaton, Bedworth, North Warwickshire and surrounding areas, living with life limiting illnesses.

Home-Start HOST provides a range of support to families in Oldham, Stockport, Tameside and Bolton, largely through volunteer home visiting support for families with a wide range of needs. Additional services include parent infant mental health support, infant feeding peer support, speech & language development and support for dads in the perinatal period.

Bethshan is situated in the heart of Mid-Wales. We run a purpose-built 40-bed nursing home catering for frail elderly people and those with dementia as well as people under 65 requiring full-time nursing care . Our first priority is to serve the needs of those from our local area.
